
An encounter form, also known as a superbill or fee ticket, is created for each patient visit. This form, which includes patient demographics, lists common services with their medical codes and a space for clinicians to record diagnoses. Front-end staff generate the encounter form, which is used to communicate the services provided during the patient’s visit. After the visit, the provider marks the appropriate services, signs the form, and, if using an EHR, may enter this information electronically.
